• I understand my testimonial may be used in any media format, including but not limited to: website, social media, print, video and digital communications.
• I understand my testimonial may indirectly reveal information about my health or medical treatment, and I consent to this disclosure for the purposes described above.
• I waive any right to royalties or other compensation arising or related to the use of the content. I hold harmless and release and forever discharge them from all claims, demands and causes of action, which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reasons of this authorization.
• I understand that I may revoke my consent in writing at any time, but materials already published cannot be retracted.

By submitting this form, you authorize Mississippi Valley Surgery Center to use your testimonial, likeness (photo/video), and related information for promotional and educational purposes in any media format, including web, social, print, video and digital communications. Submission of this form is voluntary and will not affect your medical care. You may revoke your authorization at any time by providing written notice to Mississippi Valley Surgery Center; however, materials already produced or published prior to your revocation cannot be retracted. No compensation will be provided for the use of your testimonial or likeness.
